You may send referrals using your organization’s existing referral form. Our team will review the information and manage all follow-up directly with the individual or their guardian.

Fax is the preferred method for sending referral forms and clinical documentation.

Referral Process: What to Expect

Our internal workflow ensures a smooth handoff from providers to our intake team.

1. Send Referral Information

Submit your referral via fax, phone, or secure email using your own referral form. Include any supporting clinical documentation when applicable.

2. We Review the Referral & Request Records

Our intake team will review the information provided and contact your office if additional documents or clarification are required.

3. Internal Intake & Scheduling Process

Once reviewed, our team initiates intake and prepares the patient for the next steps in their care plan.

4. We Contact the Individual or Guardian

A Pivotal Health team member will reach out directly to schedule the initial appointment and discuss expectations and timelines.
